According to Kelly and Porr (2018), healthcare has transitioned from a treat-heal-care model to a business factory type model. The new business model has placed more focus on reduced costs instead of patient outcomes. The new business paradigm is reducing expenses at the cost of health promotion (Kelly, P. and Poor, C., 2018). The healthcare industry needs to continue to adapt and change to the current environment, which is the theory of complex adaptive systems (Broome M.E., and Marshall E. S., 2020). To adapt, the healthcare industry needs to find an appropriate balance between cost-saving measures and providing the necessary care needed for the best patient outcomes. One identified way to improve both cost-saving measures and patient outcomes is to remove limitations placed on advanced practice registered nurses.
The limitation placed on these providers by state authorities limiting independent practice places more demand on the system, especially on already underserved populations (Poghosyan, L., Norful, A. & Laugesen, M., 2018). By lifting these restrictions, Chattopadhyay and Zangaro (2019), found an annual Medicare cost savings of $2.19 billion in restricted practice states and a $1.07 billion savings in reduced practice states. At a national level, this equates to over $44 billion in annual savings. To further reduce Medicare spending there also needs to be a change in the provider distribution in favor of mid-level providers. To help find a balance between cost-saving measures and the best patient outcomes, the policy at the state and federal levels should move forward in reducing or removing restrictions on advanced practice nurse practitioners.

At my current organization, employees use to have a choice of working eight-hour shifts or twelve-hour shifts. Now it is policy to work twelve hours due to workforce needs. It is also mandatory to work rotating weekends, every other Friday, Saturday, and Sunday. Other facilities that host twelve-hour shifts may not mandate every other weekend which is good for those who struggle with working three days in a row of twelve-hour shifts. This can affect employee wellbeing, leading to stress and burnout and eventually turnover. Frustration and anxiety are common feelings that may be developed by employees who become overwhelmed with work (Kelly & Porr, 2018). Jindal and Srujana’s research proved that in some circumstances, following and maintain policies may lead to errors and poor performances (Jindal & Srujana, 2020). This paper also suggest organizational well-being programs may have a positive impact on workers by showing they care about all employee health. This also encourages overall health (Jindal & Srujana, 2020).
Shortages of nurses in facilities equals an increase of need for workers which increases the workload of employees. Studies have shown more than average of workers work more than forty hours a week. Many employees work overtime to help with shortages. The workload policies have employees feeling undervalued making them question their position (Ellis, 2013). The need of the organization may not be the need of the individual as an employee and may hinder their abilities and skills in providing efficient patient care.
